New Patient Registration

Patients living within the practice area are welcome to register. For guidance, our practice areas covers Alsager, Rode Heath, Church Lawton, Oakhanger and Barthomley.

When you register with us you will need to complete the New Patient Registration Form.

Non-urgent advice: Please note

You will also be asked to fill out another form at the reception desk.

You will also be required to bring with you proof of residence i.e. recent utility bill (please note: a driving licence will not be accepted).

You will also need to complete the Cedars Medical Centre New Patient Medical Form

All new patients are offered a health check with a member of the healthcare team and we would encourage you to take advantage of this service.

Medical treatment is available from the date of registration. Please contact reception for further information.

Named Accountable GP

We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone, although every patient has a “Named Accountable GP”. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.
