Carers

What is a Carer?

A Carer is anyone, including children and adults who look after a family member, friend, neighbour who could not manage without their help. This could be due to age, physical or mental illness, addiction or disability.

Research indicates that 2000 patients on a GP list are likely to include around 200 Carers and at Cedars Medical Centre we have a process to enable these patients to be identified, registered and to ensure Carers own health needs are being met.

Are you a Carer?

If you are a Carer and would like to register this with the surgery, please complete and return a Carers Registration form.

Non-urgent advice: Please note

If we know you are a Carer, this will allow the surgery to update your medical records and for us to know that at times you are likely to be under pressure yourself.

If you would like any further information – please contact us and we will be happy to provide appropriate support and advice.
