Carers
What is a Carer?
A Carer is anyone, including children and adults who look after a family member, friend, neighbour who could not manage without their help. This could be due to age, physical or mental illness, addiction or disability.
Research indicates that 2000 patients on a GP list are likely to include around 200 Carers and at Cedars Medical Centre we have a process to enable these patients to be identified, registered and to ensure Carers own health needs are being met.
Are you a Carer?
If you are a Carer and would like to register this with the surgery, please complete and return a Carers Registration form.
Non-urgent advice: Please note
If we know you are a Carer, this will allow the surgery to update your medical records and for us to know that at times you are likely to be under pressure yourself.
If you would like any further information – please contact us and we will be happy to provide appropriate support and advice.

Social Prescribing
Social prescribing is a service provided by non-healthcare professionals called link workers. Link workers aim to support individuals with a range of social, emotional, or practical needs through referrals to non-clinical services in the community. These services can include day centres, charities, and community groups.
We are a practice that encourages our patients to increase their well-being through physical activity and healthy diets. Social prescribing is a great way to get involved in the community and participate in exciting activities that not only improve your health but also benefit the environment.
If there are nature-based activities that interest you, please speak to Social Prescriber Shellie.
Patients have already participated in the following activities:
Other support services can also be found at: https://www.cheshireeast.gov.uk/livewell/livewell.aspx
Non-NHS Services
Some services provided are not covered under our contract with the NHS and therefore attract charges.
Examples include the following:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
Please note: Passports and shotgun licences are no longer signed by the doctor.
Healthwatch Cheshire East
Healthwatch Cheshire East, part of Healthwatch Cheshire, is an independent voice for the people of Cheshire East to help shape and improve local health and social care services.
We want to hear about your views, needs and experiences to help Cheshire East get the best possible health and social care.
Travel Vaccinations
Please return your completed Travel Risk Assessment form back to reception at least 8 weeks prior to any travel date. This allows our travel nurses to have enough time to review your request and administer your vaccination course appropriately.
If you have less than 2 weeks left before travelling, we advise you to find an alternative location for your vaccinations, as our travel nurses will not be able to facilitate this request within this short timeframe.
The vaccines that are covered by the NHS and therefore do not incur a charge and are available in practice are as follows:
- Hepatitis A
- Combined hepatitis A and B
- Typhoid
- Combined hepatitis A and typhoid
- Tetanus, diphtheria and polio
The private vaccines that incur a cost are not covered by the NHS and you will be required to have them administered at another location.

Unplanned/planned pregnancy consultations are available with our GP’s.
Anyone with more complex concerns about their sexual health, will be encouraged to contact the Axess sexual health team at Eagle Bridge.
